Free PSA (Free Prostate-Specific Antigen) refers to the portion of PSA (a protein produced by the prostate gland) that circulates freely in the bloodstream, without being attached to proteins.
The Free PSA test is often used alongside the Total PSA test to improve the accuracy of prostate cancer detection, particularly when total PSA levels fall in the "gray zone" (4–10 ng/mL).
Clinical Applications:
Differentiate between:
Benign Prostatic Hyperplasia (BPH) – tends to have higher free PSA levels
Prostate Cancer – tends to have lower free PSA percentage
Helps decide whether a prostate biopsy is needed
Monitors men at higher risk of prostate cancer
