The Free Testosterone test measures the amount of testosterone that is not bound to proteins in the blood and is available for use by the body. While most testosterone is bound to proteins like sex hormone-binding globulin (SHBG) and albumin, only a small portion is considered “free” and biologically active.
This test is especially useful when total testosterone levels appear normal, but symptoms of hormonal imbalance are still present. In men, free testosterone plays a key role in sex drive, muscle mass, mood, and fertility. Low levels may indicate hypogonadism, pituitary disorders, or other health issues. In women, high free testosterone can be a sign of PCOS, adrenal disorders, or hirsutism (excess hair growth).
